SF 1413 — the Statement and Acknowledgment

SF 1413, the "Statement and Acknowledgment," is the form that connects a subcontractor to a federal prime contract and makes the labor-standard clauses apply. It has two parts: the prime fills Part I, and the subcontractor signs Part II.

Part I — Statement of Prime Contractor

The prime contractor completes this part. As the subcontractor, you should read it and confirm it matches your award before you sign Part II.

5. SubcontractorYour legal name and address — match your SAM/registration exactly.
6. Overtime clauseWhether the prime contract does or does not contain the "Contract Work Hours and Safety Standards Act — Overtime Compensation" clause.
7. Awarding firm & descriptionThe firm that awarded the subcontract, and a description of the work you will perform.
8. Project / 9. LocationThe project name and where the work is located.
10a/10b. Person signingThe prime's signer name and title.
11. Signature / 12. DateThe prime's authorized signature and the date signed.

Part II — Acknowledgment of Subcontractor

This is the part you sign. By signing, you acknowledge that the labor-standard clauses listed in item 13 are included in your subcontract.

13. Clauses acknowledgedThe printed list you are acknowledging — for example, Contract Work Hours and Safety Standards Act, Davis-Bacon Act, Apprentices and Trainees, Payrolls and Basic Records, Withholding of Funds, Copeland Act, Subcontracts (Labor Standards), and Certification of Eligibility.
14. Intermediate subcontractorsNames of any lower-tier subcontractors between you and the work, if any.
15a/15b. Person signingYour name and title.
16. Signature / 17. DateYour authorized signature and the date.
Why it matters: SF 1413 is not busywork. It is what formally brings the wage and labor-standard clauses of the prime contract down to your subcontract. Read Part I before you sign, and keep a signed copy — you will need it for certified payroll and any compliance review.
